- Monday-Friday – 8a-4p
- 40 hours per week.

The RN Case Manager will serve as primary contact person and coordinator for adults with disabilities in a group home setting in Berkshire County.
Essential Job Functions
- Provide staff training and support on medical issues and techniques as needed.
- Perform heath care assessments and develop written health care plans.
- Consult with program staff on specific medical interventions, techniques, and documentation requirements.
Qualifications
- Current Registered Nurse license with at least one year experience.
- Some experience with people with developmental challenges preferred.
- Valid driver’s license and insured vehicle
